Moving Regulations - Czech Republic
Australians intending to relocate to Czech Republic should apply for visa. They must apply personally at the Consulate General, the Embassy or any Honorary Consulate of the Czech Republic within Australia.

Every applicant has to lodge his own application. Supporting documentation must be submitted in original or certified photocopy. All statutory declarations and legal documents signatures must be witnessed and certified by a notary or a Justice of the Peace. Documents in languages other then Czech (or Slovak) must be officially translated into Czech language.

For the long stay visa the following documents must be submitted:
1. Completed and signed visa application form with two passport sized photo.
2. Travel document (passport) valid for at least 90 days after the proposed end of stay in the Czech Republic.
3. Work permit issued by the Employment Office of the Czech Republic in case of employment.
4. Evidence that the applicant will be provided with accommodation for the duration of their stay in the Czech Republic. This document has to be in or officially translated into the Czech language.
5. Health insurance policy that covers medical expenses related to disease or injury in the Czech Republic. It must include expenses connected with transportation of the foreigner to the country of which he is a passport holder. Minimum insurance cover must be 30000 EUR. If the original of this document is in English, it does not need to be translated into Czech.
6. Evidence that the applicant has no record in the Criminal record of the country where he has lived during the last 3 years. In Australia it is issued by Criminal Records section of police and it is called "Name check". It has to be in or officially translated into the Czech language.
